Extend DNS COOKIE support

The current EDNS COOKIE support does not return BADCOOKIE unless require-cookie is true. Start returning BADCOOKIE on all instances of bad server cookies.

To upload designs, you'll need to enable LFS and have an admin enable hashed storage. More information
