Document relationship between comments and user-context

This is a kind of tribal knowledge. comments were added long time ago and later we introduced a mechanism to define user-context that can contain arbitrary JSON data. As a result, comments are loaded and then converted to entries in user-context.

This should be properly explained and documented.
