Alter logging format to include thread-id by adding %t

Log4cplus logging format can emit the thread id with the addition of %t to the logging pattern. We should consider amending the default pattern, or at the very least documenting this boldly for people running MT. Otherwise diagnosing per-packet issues while running MT will be very difficult.
