Improve Hooks Libraries list in ARM left-hand column

The ARM's display of the Hook Libraries section in the left-hand column is really getting hard to navigate. With each new hook it gets worse.

  1. The library name after the number (when present) is much smaller than the numbers or topic
  2. The items are inconsistent, most have the name, then topic but not all (BOOTP vs the rest)
  3. Fonts are inconsitent (class_cmds vs cb_cmds)
  4. There doesn't seem to be an inherent scheme to the ordering beyond the order they were developed. I think alphabetical would probably be more intuitive and useful.

I also just noticed that section 16.4 appears to be out of date, neither Postgresql CB or DDNS tuning are missing from the table. Postgresql CB and MySQL CB are missing from the section listing.

It might also be nice if there were links to each hook's section in the table 16.4.

Also, this section "16.22. User Contexts in Hooks" would be better placed under 16.3 as 16.3.1, rather than at the end of the Hooks chapter itself. It applies to more than one hook and deals with their configuration.
