New form and REST commands to create new host reservation

Following the #717 (closed), we now need to create a form in the UI to specify a new host reservation and associate it with one or multiple Kea servers. Finally, when the new host reservation is submitted, it should generate appropriate REST commands to the Stork server which in turn uses the config manager to populate the host reservation to the Kea servers.
