* Catch user-library-generated exceptions.
* Remove hook registration function (no longer needed with a singleton
  ServerHooks object.
* Miscellaneous documentation changes.

a) This is now a non-copyable singleton class, and pointers
to a ServerHooks object have been removed from other classes.
b) Added a getHookName() method to the CalloutHandle class to allow
callouts to determine the hook to which they are attached. (This
change was made principally to aid writing test cases.

The hooks framework now has its own directory in "lib" instead of
living in "util".

These are called in the CalloutHandle's constructor and destructor.

And update doc and do a bit of style cleanup

It caused the pycompile script to fail

the only exposed interface is load() right now.
also made overall document cleanup/enhancements.

and detailed tests.

Admin tasks:
* Split out io_error.h from asiolink.h
* Made test files follow naming convention of main files
* More discriminatory includes in some files
* Updates tests/Makefile.am

Coding tasks:
* Add additional methods to io_socket.* and put in dummies in
  {tcp,udp}_socket.h
* Incorporated basic IO Fetch code from Scott

Though most of the tests fell into recursive_query_unittest.cc

First cut at the logging module.  Still lacking configuration and
basic appenders.

This is based on ideas done in experiments/jreed-layout, but
I manually did this again.

Moved around directory structures.
A single python subdirectory under src/lib for single "isc" module.
No "cpp" directories.

No pyshared or includes symlink trees.

I tested running the c++ tests and python tests.
I tested running run_bind10.sh from source tree (with nothing installed).
I tested running bind10 from installed (after moving source).
